Registered Nurse - Large/Specialty Hospital Supervision & System Flow Out of Province Liaison

The programs and services in these areas predominately involve expert knowledge and work independently and collaboratively in a supervisor or system coordinator function. They provide leadership and are responsible for care coordination, patient prioritizing and flow within the available resources within facility, provincial or interprovincial health services. They are required to remain current in best practice and act as a resource for other health care providers.

These positions follow and support patients through health systems outside of Prince Edward Island to encourage timely and appropriate return to their home province and ensure efficient use of out-of-province health resources. The RN will coordinate health services when patients return to their home Province (PEI) as needed.

Leadership and Clinical Practice:

  • Responsible for care coordination, patient flow as well as collaborating with and prioritizing the available resources within facility, provincial and interprovincial health services.
  • Coordinate all patient admissions and transfers (in and out of Province).
  • Work with Island EMS and Medacom Atlantic to develop and implement appropriate and responsive inter-facility transfer capabilities
  • Establish and foster effective working relationships; models strong communication and teamwork skills to achieve system improvements among divisions, multiple facilities and care providers
  • Optimize and manage resources in the most efficient and effective manner
  • Use current literature/research and performance indicators to support evidence-based practice
  • Proactively seek professional development opportunities for themselves and others
  • Attend and/or participate in programs to reinforce nursing education and to remain current with developments and changing trends in the nursing field
  • Maintain a psychologically and physically safe environment and report all safety hazards
  • Identify clinical practice issues affecting patient safety qualify outcomes and implement best practice patient care standards
  • Document care activities consistent with policy, system requirements and scope of employment
  • Follow and adhere to all policies, ethical and legal guidelines

Statement Of Qualifications:

  • Baccalaureate Degree in Nursing or Psychiatric Nursing with extensive supervisory/leadership experience (5yrs +) in the area of clinical practice
  • Diploma in Nursing supplemented by a post-secondary Nursing leadership and healthcare management program and extensive supervisory/leadership experience will be considered. (Current incumbents will be considered)
  • Extensive experience in complex and team/unit-based care coordination including patient flow
  • Must have knowledge of health care systems in Prince Edward Island, Nova Scotia and New Brunswick including relevant legislation, programs and services.
  • Active or eligible for registration with the Nova Scotia College of Nursing.

Salary Range: $44.32 - $55.45 per hour (RN4)

Bi-Weekly Hours: 75 Hours Bi-Weekly / Mon - Fri
