Role Description
This is a full-time on-site role for a Care Coordinator, based in Montreal, QC. The Care Coordinator will be responsible for organizing and managing care plans, ensuring that clients receive the appropriate services in a timely manner. Daily tasks include coordinating with clients, families, and service providers, managing case files, and maintaining clear communication to ensure seamless delivery of care solutions tailored to each client's needs.
Qualifications
- Experience in Care Coordination and Case Management to oversee and implement effective care plans
- Strong Communication skills for engaging with clients, families, and service providers
- Knowledge and experience in the field of Home Care services
- Customer Service skills to provide empathetic and professional support
- Organizational and problem-solving skills to handle diverse client needs and prioritize tasks effectively
- Proficiency in documentation and record-keeping systems
- Fluency in English and French (bilingualism is considered an asset)
- Healthcare experience or a related certification is considered an advantage
