This posting is for a temporary full-time position until December 2028.

The Community Outreach Navigator provides community-based, participant-centered support to individuals experiencing houselessness or housing insecurity. Through outreach, case management, and system navigation, this role supports individuals in accessing services, building stability, and working toward their goals. Working collaboratively with internal teams, RCMP, Enforcement Services, and community partners, this position plays a key role in delivering coordinated, wraparound supports using a trauma-informed and strength-based approach.

Key Responsibilities:

  • Provide outreach and case management support to individuals experiencing houselessness or housing insecurity
  • Assess participant needs, complete risk assessments, and support development and follow-through of intervention plans
  • Connect individuals to housing, financial supports, mental health, addiction services, and other community resources
  • Build trusting relationships through consistent, community-based engagement (in-field, office, and virtual)
  • Collaborate with internal teams, RCMP, Enforcement Services, and community agencies to coordinate services and reduce barriers
  • Advocate for participants and facilitate referrals, including warm hand-offs to service providers
  • Maintain accurate and timely documentation, case notes, and records in electronic systems
  • Participate in case conferences, community tables, and ongoing service coordination efforts
  • Support program delivery across the Human Services Innovation Branch as required

Qualifications and Skills:

  • Degree in Social Sciences with a minimum of 4 years related experience, or Diploma with 6 years of related experience
  • Minimum 2 years’ experience working with vulnerable populations and navigating social service systems
  • Experience supporting individuals experiencing houselessness, mental health challenges, or crisis situations is an asset
  • Knowledge and/or training in risk assessment, safety planning, mental health, addictions, and family violence
  • Strong understanding of community resources, service systems, and referral pathways
  • Ability to build relationships and work collaboratively with diverse stakeholders
  • Strong communication, problem-solving, and conflict resolution skills
  • Ability to work independently in dynamic, community-based environments
  • High level of adaptability, resilience, and professional judgment
  • Registration with a regulated professional body (e.g. ACSW) is considered an asset

Conditions of Employment:

  • Ability to obtain RCMP Enhanced Reliability Security Clearance
